Abstract Background Regardless of the cause superficial siderosis (SS) disease, which is bleeding, source bleeding cannot be found in some cases. Case presentation In this article, we report two cases with idiopathic SS. 1 presented bilateral hearing loss, cognitive impairment, sleep disturbances, and tremors. 2 sensory neural ataxia, spastic paraparesis. both cases, brain MRI indicated evidence CT myelogram SPECT labeled RBC couldn’t help finding occult bleeding. Conclusion SS a rare central nervous system disease caused by deposition hemosiderin spinal cord, results progression neurological deficits. The hemorrhage often subarachnoid haemorrhage, intracranial surgery, carcinoma, arteriovenous malformation, nerve root avulsion, dural abnormality. condition progresses slowly and, time diagnosis confirmed, damage irreversible. our clarified definitive diagnosis, but could not find should considered ataxia even if no found.
